What C_Verified checks, how review works, and where the product stands today. Start with the questions that matter to your team.

A checkpoint before the chart changes.

Before an AI-generated note, order draft, image, or file can move toward the medical record, C_Verified is designed to apply deterministic checks and the healthcare organization's policy.

In the intended product model, the result is a policy outcome with a reason attached. That outcome controls the requested action. It does not certify that the medicine is correct.

Verified
In the intended product model, the request meets the required policy conditions and can continue with decision evidence attached.
Held
In the intended product model, a person, permission, or required piece of evidence is needed before the request can continue.
Denied
In the intended product model, a required policy condition was not met, so the requested action stops.
C_Verified is designed to check the action. Clinicians judge the medicine.

It does not diagnose, recommend treatment, or decide whether clinical content is true.

The request has to explain itself.

C_Verified is being designed around five clear questions. The healthcare organization decides what evidence and authority each action requires.

  1. Identity

Which AI, system, organization, and route are behind the request?

  2. Authority

Is this requester allowed to take this action in this context?

  3. Evidence

Are the sources and supporting items required by policy present and resolvable?

  4. Integrity

Did the content or file change between the request, decision, and handoff?

  5. Policy

The intended policy path can return Verified, Held, or Denied.

C_Verified comes first.

Open C Health Systems is building the control layer before expanding the intelligence layer. The sequence keeps policy, evidence, and human authority at the center.

Topic 01

C_Verified

What is C_Verified?

C_Verified is a policy-controlled decision point at the boundary between an AI request and an action that could affect the medical record. It is being built to apply the healthcare organization's rules before that action moves forward.

Where does C_Verified sit?

At the action boundary between an AI system and the record-related action it is requesting. The exact connection depends on the use case and system architecture. C_Verified is being designed to operate independently of any one AI vendor.

Is C_Verified an EHR?

No. C_Verified does not replace the electronic health record. It is being designed as an independent control point for AI actions that could affect the record.

What does C_Verified check?

It is being designed to check identity, authority, required evidence, content and file integrity, and the organization's deterministic policy. Those checks concern the requested action, not the truth of the medicine.

Does C_Verified decide whether clinical content is correct?

No. C_Verified does not diagnose, recommend treatment, or certify that clinical content is true. It is designed to check the action around the content. People retain clinical judgment.

Who decides the rules?

The healthcare organization does. C_Verified is being built to apply the organization's adopted policy. It does not invent clinical policy or replace organizational authority.

What stage is C_Verified in?

C_Verified is in active development. Open C Health Systems is defining design-partner use cases and the first AI action paths healthcare organizations need to control. Product capabilities, timing, and availability may change.

Topic 02

Outcomes and decision evidence

What outcomes is C_Verified being designed to return?

In the intended product model, Verified means the request meets the required policy conditions and can continue. Held means a person, permission, or required piece of evidence is needed. Denied means a required condition was not met and the request stops. None of these outcomes certifies clinical correctness.

What happens when a request needs human review?

In the intended product model, the policy path can return Held and route the request to the appropriate reviewer. The reason for the hold should travel with the request so the person knows what needs attention and what happens next.

What is a C_Verified decision receipt?

A decision receipt is designed to connect the request, the policy version, the evidence considered, the outcome, and the reason in one tamper-evident record. It supports later inspection without claiming that the clinical content was correct.

What does tamper-evident mean?

It means the evidence is structured so later changes can be detected. It does not mean that alteration is impossible, and it does not replace the organization's security, retention, or legal processes.

Does a decision receipt determine legal responsibility?

No. A receipt can preserve evidence about the request, policy path, review, and outcome. It does not decide legal fault, remove liability, or replace legal advice.

Topic 03

Open C Health Systems

What is Open C Health Systems?

Open C Health Systems is the company building C_Verified and developing the broader Autonomous Health Intelligence roadmap.

What is the relationship between Open C Health Systems and C_Verified?

Open C Health Systems is the company. C_Verified is its first product and the first policy and evidence layer in active development.

What is Autonomous Health Intelligence?

Autonomous Health Intelligence names the broader Open C Health Systems platform roadmap for future clinical workflow and record products. Those broader products remain under development. Features, timing, and availability may change.

Is Open C Health Systems building a general AI chatbot?

No. The first product focus is the control and evidence layer around AI actions in healthcare, not a general-purpose chatbot.

Who is Open C Health Systems building for?

The product direction is focused on healthcare organizations evaluating how AI-generated content and actions can move toward the medical record, including hospitals, clinics, and health systems.

Topic 04

Key terms

What is an AI action boundary?

It is the point before an AI request can take an action that affects another system, workflow, or record. C_Verified is focused on the boundary before an AI action could affect the medical record.

What is deterministic policy?

It is policy applied through explicit code and rules so the same verified inputs produce the same policy outcome. The decision path is not delegated to another AI model.

What is content integrity?

Content integrity checks whether the content or file stayed consistent across the request, policy decision, and handoff. It does not determine whether the clinical meaning is correct.

What is human review?

Human review is a policy-required decision by an authorized person before a Held request can continue. The organization defines which actions require it and who can perform it.

What is a cryptographic hash?

A cryptographic hash is a digital fingerprint for data. If the underlying data changes, the fingerprint changes, which can help reveal later alteration.
